Quarterly reference period, with substantial reporting lag; quarter start is not publication time. Extraction date and submission count remain in the full CSV receipt. Checked once per 24 hours, health every 15 minutes; stale after 365 days from reference-quarter start. Not an insurer-specific rating or insurance quote.
